Seeing a doctor to keep you healthy shouldn’t drain your healthcare budget.
You can protect your pocketbook and keep your out-of-pocket healthcare costs
to a minimum when you enroll in the NALC Health Benefit Plan.
In 2010, you’ll continue to pay just $15 for office visits, consultations and routine
physical exams, including school, camp and sports physicals, when you use a
PPO provider. The initial office visit associated with a routine colonoscopy or
sigmoidoscopy is also covered.
Covered lab tests associated with a routine physical, are also paid in full. We
pay 100% for all other medically necessary lab charges when you use Quest
Diagnostics or LabCorp.
Maintaining good health for you and your family is important to the Plan. Our
preventive care benefit for adults includes 100% coverage for routine adult
screenings, such as mammograms, osteoporosis and colorectal screenings,
routine pap and chlamydial testing, diabetic and PSA testing, and certain adult
routine immunizations endorsed by the Centers for Disease control and
Prevention (CDC), when performed by a CIGNA PPO provider. When the NALC
Health Benefit Plan is your primary carrier, you can get your pneumococcal,
influenza, or H1N1 vaccine free at a local NALC CareSelect pharmacy. Click

Preventive care for children has been expanded to cover screening tests for
congenital hypothyroidism, PKU and sickle cell. A routine pap or chlamydial test,
as well as a routine urinalysis, are now covered for dependent children.
When you receive inpatient care in a PPO hospital, charges are paid in full after
you pay a $200 copayment per admission. And new for 2010, we now pay the
Plan allowance for non-PPO pathologists, radiologists, and emergency room
physicians at the PPO benefit level when the services are rendered at a PPO
hospital.
With over 1,200,000 participating providers nationwide, chances are your
current provider is already a member of our CIGNA HealthCare Shared

If you become sidelined due to a minor injury, our accidental injury benefit
includes 100% coverage for fracture care, suturing of simple lacerations and
immobilization by splinting or strapping of a strain or sprain, when you receive
care within 72 hours of the accident.
In addition to covering treatment of an injury to a sound natural tooth under our
Dental benefit, we now cover removal of impacted teeth that are not completely
erupted.
When Medicare Part A, Part B or both are your primary payer, the Plan will
continue to pay in full the balance after Medicare for any expense that Medicare
covers.
When a generic medication is appropriate, ask your physician to prescribe a
generic drug from our NALCSelect generic list. The amount you pay for a 90-
day supply of an NALCSelect generic medication purchased through our mail
order program is $5.00 and only $4.00 when Medicare Part B is your primary
payer. A 90-day supply of all other prescription generic medications costs just
$12.00 ($10.00 when Medicare Part B is primary) when purchased using our
mail order program.
Our Plan continues to receive some of the highest possible ratings by OPM
Quality Assurance standards and OPM Customer Satisfaction Surveys. In
2010, the NALC Health Benefit Plan will once again provide highly competitive
premiums, comprehensive benefits and prompt and courteous claim service to
